Geriatric Care Management Services

There is just so much information involved in formulating and carrying out care plans that the advice of a professional is needed make plans work. When seniors are impaired in their ability to participate in care planning, the task becomes impossible without specialized help.

A professional Geriatric Care Manager (GCM) is a health and human services specialist who helps families who are caring for older relatives. The GCM is trained and experienced in any of several fields related to care management, including nursing, gerontology, social work, or psychology. 

The most common source of care planning complications are behavioral health issues. When a family member suffers from Depression or Dementia they acquire specialized care needs as they lose the ability to express preferences in a way that is useful in making choices.  Why not meet this challenge my getting advice from the same people that educate the geriatric care community on these issues?
